Responsibilities
Collects quality blood specimens, including maintenance of iSTAT competency for quality POC testing if applicable. Expected performance to include less than 2.5% blood culture contamination rate, less than 1.0% patient recollections due to unsatisfactory specimen integrity and no more than 1 patient complaint regarding non-adherence or compliance of technical and/or behavioral standards.--------------------------------------------------Ensures accurate patient identification. Expected performance is demonstrated by having zero rejected blood bank specimens, less than 2 correctable blood bank-labeling errors, zero misidentified patient incidences and zero mislabeled specimens.--------------------------------------------------Facilitates effective communication at the beginning and end of work shifts, by the use of clear and concise verbal discussion and engaged behavior to ensure proper continuum of patient care. Demonstrates appropriate hand off exchange, including the clearing of all pending orders before leaving work station and/or providing clear explanation of work to be done. Performance of no more than 5 instances of unsuccessful hand off events per year is expected.--------------------------------------------------Maintains supplies (to include, but not limited to phlebotomy baskets, carts, workspace and electronic and telecommunication devices). Expected performance is demonstrated by having no more than 1 expired tube found in carts or baskets, performs monthly cart checks as assigned and ensures phlebotomy supply area is clear of expired products, clean, organized and restocked daily. Electronic devices are to be kept in appropriate condition and are to be turned over to next shift in working order.--------------------------------------------------Utilizes lab/hospital systems in order to accurately document lab activity to include collection lists, pending logs, un-received reports, and labels according to established site protocols throughout the shift to ensure established turnaround times are met. Expected performance is demonstrated by having less than 1 delayed sample per month.--------------------------------------------------
Qualifications
EDUCATION:Required - High School diploma or equivalent.Preferred - Graduate from an accredited phlebotomy program.
EXPERIENCE:Preferred - A minimum of 6 months to 1 year, full time experience in a hospital setting.
LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIREDBLS (CPR)-American Red Cross or AHA Healthcare Provider Current Phlebotomy certification from one of the following approved agencies:ASCP (American Society for Clinical Pathology)ORASPT (American Society of Phlebotomy Technicians)ORNHA (National Healthcareer Association)Also, certification as a Clinical Laboratory Assistant or Medical Laboratory Assistant by ASCP is acceptable.PREFERRED
