Cancer Clinical Care Coordinator at NHS

Job Descrption
We are recruiting a Cancer Clinical Care Coordinator to work within the Nurse Specialist team who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

As part of a 2-year Pilot, the Macmillan Cancer Care Coordinator (MCCC) will work alongside the Clinical Nurse Specialists in Skin and Lung cancers to ensure a safe and efficient service for patients. You will contribute towards the provision of providing a high standard of evidence-based care, supporting the management and supervision of patients throughout their treatment pathway.
Main duties of the job
• Provide a seamless service to patients with proximal supervision from the tumour specific Clinical Nurse Specialist (CNS).
• Coordinate care by providing a single point of access, including rapid re-entry into the system for those patients identified as having urgent or specialist needs.
• Communicate effectively within the multidisciplinary team and other departments to... ensure that patient care is well planned and coordinated.
• Have contact face to face and telephone communications with patients, carers and health care professionals.
• Contribute to the improvement of services.
• It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical team with the focus on patient needs.

Job responsibilities

ENHT is seeking to maintain and expand the Mount Vernon Cancer Centre staff as we move to an exciting new future. We welcome applications from those who would relish the opportunity to be part of new and developing cancer service, building on the excellent reputation, research and patient experience the Cancer Centre already delivers.

In addition, you will receive a High Cost Area Supplement of 15% of your basic salary, subject to a minimum payment of £4,314 and a maximum payment of £5,436 per annum pro rata.
Person Specification
Previous Experience
• Evidence of continued role development.
• Experience in or a good understanding of health and social care services and pathways.
• Experience of multi-professional working.
• Experience of coaching/teaching patients and carers.
• Understanding of Macmillan Cancer Support and its role across the UK.
• Experience of coordinating a patient workload.
• Knowledge of relevant cancer treatments, interventions, and terminology.
• Awareness of national cancer strategies and policies.
Qualifications / Training
• GCSE English Language and Mathematics at Grade A-C or equivalent.
• NVQ4 or NVQ3 plus training to diploma level equivalent, or equivalent through short courses.
• Oncology experience/training or experience/training in long term Health Conditions.
• Evidence of continued role development.
• Coaching or teaching qualification.
• Macmillan Learning and Development Courses.

£25,147 to £27,596 a year per annum, plus 15% High Cost Area Supplement.
