We are looking for a highly motivated person who would like to join our exciting team as a Patient Pathway Coordinator for the General Surgery POD here at Newham University Hospital. This is a substantive role which is full time (37.5 hours). Agile working arrangements can be provided to fit around other members of the team. You will be working within a department but allocated to a specific specialty. The successful candidate will be scheduling Outpatient appointments for that service, as well as monitoring that patient's journey within that pathway.

We are looking for individuals with CRS Cerner Millennium knowledge and have job experiences of working in a busy, fast-paced division. Experience of... working in administrative offices is not essential but preferable.
Main duties of the job
To be the point of contact for patients for all issues relating to their pathway of care, both for inpatients and outpatients.
The post holder will work as part of a multi-disciplinary team (POD), under the direction of the Clinical Lead, and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.
To provide a consistent approach across the Trust, Standard Operating Procedures (SOP) will describe how the functions of this role will be carried out along with the timescales that will be adhered to.

Person Specification
Experience
~ Minimum of 3 years medical secretarial/pathway administration experience.

Skills
Proactive with the ability to use own initiative.
Able to present a positive image of her/himself and the service.
Ability to self-reflect, carry out tasks of own job and identify what s/he needs to learn to do current job better.
Ability to take an active role in agreed learning activities and keep a record of them.
Knowledge of national targets with respect to the patient pathway.
A sound knowledge and demonstrable competence regarding 18-week referral to treatment rules.
To have a sound understanding of NHS-wide initiatives and experience of responding positively to ensure local implementation.
Ability to provide constructive opinions and views on ideas for service improvement.
Ability to use and maintain resources efficiently and effectively and encourage others to do so.
Acts responsibly as a team member and seeks help if necessary.
A proven record of being able to work within set timeframes working to priorities and deadlines.
Able to monitor quality of own work and others.
Ability to produce and present written reports and correspondence of a high calibre.
Competence with a range of IT packages, e.g., CRS.
Methodical and logical approach to work prioritisation and planning.
Flexible approach to the planning and coordination of patient pathways to ensure maximum use of clinical time.
The ability to plan and coordinate activities and changes to systems or standards.
Knowledge
~ Previous Validating Experience within the NHS/Experience of Managing Patient Pathways in line with RTT (Referral to Treatment) reporting.

Qualifications
GCSE A Level or equivalent standard of Education.
AMSPAR or equivalent e.g., a good level of medical terminology.
ECDL or equivalent experience e.g., experience of a range of IT packages such as Word, Excel, Outlook, and PowerPoint.
Pathways in line with RTT (Referral to Treatment) reporting.
Other
Has a flexible approach and attitude.
Adapts to changing circumstances.
Ability to treat everyone with whom s/he comes into contact with dignity and respect.
Experience of Human Rights, Equality & Diversity policies and procedures.
Highly developed communication skills, with experience of dealing with patient's anxiety in a reassuring manner. An ability to communicate professionally at all levels.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
£31,081 to £33,665 a year per annum (pay award pending).
