Job Summary:

Camden and Islington NHS Foundation Trust is seeking a highly motivated and experienced Specialist Pharmacist to join our dedicated team on a full-time basis. As part of our rotational Band 7 program, you will have the opportunity to provide comprehensive medicines management and optimisation services to patients and healthcare teams across our clinical rotations, including Medicines Safety and Governance.

In this role, you will support the Lead Pharmacists in the development and delivery of high-quality clinical pharmacy services. You will also play a key role in supervising and mentoring junior pharmacists and trainee pharmacists, contributing to their growth and development.

Main duties of the job
• Deliver a safe, effective, and patient-centred clinical pharmacy service... across designated rotations, including reviewing prescriptions, identifying drug interactions, and supporting patient adherence.
• Provide expert clinical pharmacy input at ward level and within community mental health teams, contributing to the multidisciplinary care of patients.
• Support the development of pharmaceutical care plans, manage adverse effects, and ensure timely and accurate medication supply, including during patient discharge.
• Contribute to the Trusts Quality Improvement (QI) and Medicines Audit programmes, identifying and undertaking QI projects and audits that improve service delivery and patient care.
• Participate in the on-call pharmacy service, including weekend and bank holiday dispensary rotations, providing advisory support as required.
• Offer training and support to trainee pharmacists and Band 6 pharmacists, playing an active role in their education and development.
• Provide input into Trust-wide policies and procedures concerning medicines use and work closely with the governance teams to ensure compliance and safety.

• A proactive approach to problem-solving, with the ability to critically appraise clinical evidence and make informed decisions to optimise patient outcomes.
• Commitment to continuous professional development and a desire to contribute to innovative projects within the Trust.
