Nurse Practitioner at AgeCare

Job Descrption
The Nurse Practitioner (NP) at AgeCare works collaboratively with the Director of Care and an interdisciplinary team to provide quality, safe resident care. The NP promotes a culture of trust, respect, teamwork, and quality, ensuring the rights and individuality of residents, families, and staff are upheld.
What You Will Do
• Collaborate with the MD, DOC, and care team.
• Assess, diagnose, and manage resident health conditions.
• Advocate for residents and engage with families and substitute decision makers.
• Participate in resident care conferences.
• Lead clinical team in care activities, including hospital discharges, palliative care, and dementia management.
• Identify and address trends in resident care, guiding educational programming.
• Promote best practices and mentorship among clinical staff.
• Develop and deliver education to staff, residents, and families.
• Document clinical data and care plans promptly.
• Support a safe and quality-focused... organizational environment.
• Participate in Quality Committee meetings and CQI initiatives.
• Provide after-hours and on-call coverage.
What You Bring
• Minimum one year of experience in geriatric, rehabilitation, or long-term care nursing.
• Leadership experience is an asset.
• Certifications in gerontology, mental health, behavioral support, or palliative care are an asset.
• Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class and entitled to practice.
• Knowledge of relevant Long Term Care healthcare acts and standards.
• Availability for on-site presence.
