Medicus Health Partners is the largest GP Partnership in Enfield, delivering services to over 93,000 patients from its 9 locations.

We currently employ over 250 staff members, with one-third dedicated to clinical and two-thirds to non-clinical workforce. Within our practices across London Enfield. We are seeking a full time Health Champion Co-ordinator for MHP - Enfield Unity Primary Care Network.

Enfield Unity Primary Care Network is the second largest PCN in England representing over 158,000 patients across 17 GP Practice sites. Our aim is to improve services and outcomes for patients by supplying a well-trained, motivated workforce.

Health Promotion Case Worker & Co-ordinator

Health Promotion Case Worker & Co-ordinator have an important role to play in helping people in their community to make positive changes to their health and wellbeing by encouraging others to make healthy lifestyle choices. Enfield Unity Primary Care Network are looking for an experienced Health Champion... to coordinate our health inequalities work and support a network of Health Champions across the Enfield voluntary sector.

Overview

Our Health Promotion Case Worker & Co-ordinators will have an understanding of all areas of health improvement:

Physical activity – Signposting people to gyms, community and online exercise classes

Healthy eating – Such as introducing people to healthy cookery classes in their area or encouraging their workplace to provide more fruit and vegetables for employees

Heart Health- Support a healthy lifestyle to encourage better cardiovascular health including blood pressure, pulse checks and smoking cessation.

Cancer Screening- Explaining, promoting and myth busting concerns around screening programmes.

Vaccination & Immunisations- Explaining, promoting and myth busting concerns around vaccinations and Immunisations.

Mental health support – Direct people to mental health support services and work with people on their mental resilience through practices like mindfulness

Signposting to local services – Develop an in-depth knowledge of services to direct people to such as sexual health support, smoking cessation resources or drop-in centres.

Main Duties

Networking- Develop the local Health Champions network, support them in achieving the PCNs reducing health inequalities aims and objectives.

Health Coaching- Provide motivational coaching and education to promote healthy eating, accessible exercise, smoking cessation and other healthy lifestyle choices. Conduct both one-on-one sessions and group sessions, both in person and remotely.

Social Prescribing- Take referrals from PCN practices, assessing individual patients and managing a patient caseload, providing signposting to different services and connections with community groups and services. These services may include befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering, forging working relationships and creating referral pathways with VCSE organisations and many more.

Inequalities in health – Understand local health inequalities and help to develop policies and strategies for addressing these.

Effective communication- Support health messages and encourage local Health Champions to reach their community to make positive changes to their health & wellbeing.

Organise – Health Champion training, meetings and health promotion events in the community.

Specific Duties The duties and responsibilities may include any or all of the items in the following list.

1. Provide Health Coaching and Social Prescribing support to patients experiencing deprivation and/or with Long Term health conditions.

2. Coordinate and facilitate Health Champions network meetings, training and events.

3. Provide support, information, training and guidance to Health Champions on the various health promotion areas of focus as directed by the PCN Inequalities Lead.

4. Attend community venues and events to promote various health promotion activities.

5. Co-design health improvement programmes, materials and training.

6. Conduct blood pressure and pulse checks in the community, provide health lifestyle advise, signpost to local health and social prescribing services.
• Utilise population health intelligence to proactively identify areas of inequality and work with a cohort of patients with long term health conditions to deliver personalised care.

8. Support patients to utilise decision aids in preparation for a shared decision making conversation.

9. Support people to take up training and employment, and to access appropriate benefits where eligible.

10. Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

11. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.

12. To be aware of the PCN philosophy and to treat people with dignity and respect, offer constructive support to colleagues and the general public.

13. To be responsible with other members of the team for maintaining and monitoring the PNC & practice systems

14. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

15. Inform the lead GP of any concerns regarding the role and any professional development needed.

16. Be aware of own professional boundaries and what to do when you have reached them.

17. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team’s performance.

18. Promoting and maintaining their own and others’ health, safety and security as defined in the PCN Health & Safety Policy.

19. Computer data entry/data allocation and collation; processing and recording information in accordance with procedures

20. Providing clerical assistance to PCN staff as required from time to time, including word/data processing, filing, photocopying and scanning.

21. Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers

22. To deal with delegated tasks efficiently, according to guidelines set, and reporting regularly any problems and targets reached as required.

23. To be aware of all procedures and work in accordance with written protocols

24. Understand the principles of shared decision making and work in a person-centred way

25. Understand and maintain infection control procedures

26. Understand and work within GDPR and good practice guidelines for information handling, including patient confidentiality.

27. Work flexibly according to the needs of the service and provide cover for absent colleagues.

28. Assist and supervise more junior members of staff to ensure effective and safe practice.

29. Attend PCN and other meetings as required, both on-line and face to face.

30. Attend other sites as directed to carry out any tasks or services relevant to this role.

31. Undertake mandatory and additional training as identified by the PCN.

32. Any other duties, appropriate to the grade, as required by the service manager or Lead Clinician.

33. Duties may be varied from time to time under the direction of the Lead Clinician/ service Manager, dependent on current and evolving workload and staffing levels

Qualifications

Essential: Recognised professional qualification in social work, health, education or equivalent, and evidence of a commitment to continuing learning and professional development.

Desired: Educated to degree level. Level 2 Award in Understanding Health Improvement or health coaching equivalent.

Essential: Safeguarding Certification

Essential (or will obtain)- Training as set out by the Personalised Care Institute and shared decision making

Essential: Training in motivational coaching and interviewing

Experience

Essential: Experience of working in a people-centred environment.

Desired: Experience of supervising and developing volunteers, providing effective leadership, managing conflict and using negotiation skills where necessary.

Essential: Experienced in Primary Care, Community Care, Social Work or Public Health with demonstrable experience in Coordinating Teams.

Desired: Experience of establishing and sustaining a broad range of professional partnerships and engaging, as well as consulting, with service users and stakeholders through effective collaboration and communication skills.

Essential : IT Skills (MS Office) and Outlook email and experience of using database software.

Essential: Excellent interpersonal skills

Job description may be amended following consultation with the post holder, to facilitate the development of the role, the practice, and the individual.

Job Types: Full-time, Fixed term contract
