🗓 Monday to Friday | 🕘 09:00-17:00 | 💷 Up to £30.00 per hour
We're seeking an experienced and dedicated Senior Social Worker (Adults) to join our team in Stafford. If you're passionate about facilitating safe, timely discharges and supporting adults across care settings - we want to hear from you!
✨ Role Details
👩⚕️ Band 6 Qualified Social Worker - 37.5 hours per week
⏰ Start time: 08:00-17:00 (1-hour lunch)
📍 Base locations:
Cannock Specialist Centre (Cannock)
Stafford Specialist Care Home Centre (Stafford)
➕ Flexibility required to support additional homes in Cannock / Seisdon / Stafford areas
📅 Contract: 4 months (possible extension)
👩💼 Line Manager: Vanessa Cotton - Operational Service Delivery Lead, Discharge to Assess West
🧡 About the Role
You will be joining the West D2A Beds Discharge Team, supporting timely discharges and helping prevent delays for Staffordshire patients placed in Discharge to Assess (D2A) beds across private care homes commissioned by MPFT.
You'll work closely with a Multi-Disciplinary Team, including:
Social Workers
Social Care Assessors
Nurses
Occupational Therapists & Physiotherapists
MPFT Home First Team
This role requires a flexible, confident professional who can build strong partnerships across hospital and community settings.
